Who should you call if you have a question about your medication:
You have a right to request counseling from one of our pharmacists. Our pharmacists are here to counsel you and answer questions that you may have about your prescription, and we are always happy to help. Simply call the pharmacy toll free at 888-348-3010. How to learn about the cost of my medication:
Upon receipt of a prescription and with any changes, we will contact you to inform you of the network status of our pharmacy, your out-of-pocket costs including copays, deductibles and coinsurance, and the cash price of the medication upon request.
How to place a prescription order, refill or check on a prescription status, including order delays:
To place an order for a new fill, refill or check on prescription status, please call our toll free number at 888-348-3010 to speak with a pharmacy representative. Information on prescription substitutions:
Prescriptions will be filled as written by prescriber. The pharmacist will work with the patient and prescriber to ensure the most cost-effective medication is dispensed.
How to transfer a prescription to another pharmacy:
You have the right to transfer the prescription to another pharmacy of your choice. If you would like to transfer a prescription to another pharmacy, please contact the pharmacy toll free at 888-348-3010. How to obtain medications not available at the pharmacy:
If the requested medication is not available at the pharmacy, the pharmacy will work with the patient to locate a pharmacy which has the medication in stock and that can dispense it. Our pharmacy team will then transfer the prescription to that pharmacy.
How to handle medication recalls:
If we receive a notice of a recall that requires patient-level notification and/or action, we will contact you using the contact information we have on file for you. If you suspect that you have a recalled medication, please contact the pharmacy at toll free at 888-348-3010 to speak with a pharmacist. How to handle suspected medication issues, errors or adverse reactions:
If a medication issue, error, or adverse event is suspected, please contact the pharmacy toll free at 888-348-3010. How to communicate effectively with the pharmacy:
Our pharmacy is able to provide options for language translation, alternate communication materials, or communicate with approved caregivers upon request.

Please let us know any changes in your allergies, health conditions, any nonprescription or herbal products you are taking, and your contacting and billing information by calling the pharmacy toll free at 888-348-3010. How to report a concern or complaint:
You have a right and responsibility to inform us of concerns, dissatisfaction, or make complaints about the services you did or did not receive without fear of retaliation or interruption of services. If you have a complaint, please call us. You will receive notice (written or verbal) of the complaint resolution within 14 business days. If needed, you may contact your State Board of Pharmacy, the Arizona State Board of Pharmacy (our home state pharmacy board), or our accreditation organization(s).
We have a plan if a disaster occurs:
Disasters include a fire to our facility, chemical spills, major weather events and evacuations at or of our facility or in your local area. Our goal is to continue to service your needs if a disaster is anticipated or occurs. If there is a threat of disaster, or severe weather, or evacuation in your local area, please contact us for any medications you need to make sure you have enough. Follow directions from the authorities in your local area. We will use every resource available to continue to make sure we can serve you. While unlikely, there may be times where we cannot meet your needs because of an emergency. In these situations, you must use your local rescue or medical facility. If we cannot get your medication to you due to an emergency, we will transfer your medication to a local pharmacy of your choice. If we cannot reach you or you cannot reach the pharmacy, please listen to your local news for help. Please make sure we have an emergency contact number so that we can reach you.
SAFE DRUG DISPOSAL
FIRST CHOICE: DRUG TAKE-BACK EVENTS
To dispose of prescription and over-the-counter drugs, review the medication information insert supplied with your order or call your city or county government's household trash and recycling service and ask if a drug take-back program is available in your community. Some counties hold household hazardous waste collection days, where prescription and over-the-counter drugs are accepted at a central location for proper disposal. SECOND CHOICE: HOUSEHOLD DISPOSAL STEPS
Take your prescription drugs out of their original containers.
Mix drugs with an undesirable substance, such as cat litter or used coffee grounds.
Put the mixture into a disposable container with a lid, such as an empty margarine tub, or into a sealable bag.
Conceal or remove any personal information, including Rx number, on the empty containers by covering it with permanent marker or duct tape, or by scratching it off.
The sealed container with the drug mixture, and the empty drug containers, can now be placed in the trash.

A prescription reader is a device designed to audibly convey labeling information. Upon the patient or their caregiver's request and if suitable to address the patient's visual impairment, a prescription reader will be made available to patients for the duration of the prescription.
